Insurance FAQ



Please check with your insurance company about specific questions; these are only guidelines and CCBS does not assume any responsibility for the accuracy of these statements as they pertain to your plan. Only your insurance company can answer your questions, and you are responsible for understanding the inclusions and limitations of your plan.

          Amount billed: the amount charged by the health care provider or facility.


         Coinsurance: a percentage of covered expenses you pay after you meet your deductible.


         Copay: a flat fee you pay for certain covered services such as doctor visits or prescriptions.


         Deductible: the amount you need to pay each year before your plan starts paying benefits.


        Discount: the amount you save by using an in-network provider.


        In-network provider: a group of health care professionals who offer discounts to you based on their relationship with your insurance provider.


  •     CCBS is in network with: Aetna, BabyNet, Beacon, Blue Cross Blue Shield, Companion Benefits Alternatives, Cigna, Coventry, Optum, Tricare, United Healthcare


        Out-of-network provider: health care professionals that do not belong to your network, you may be able to use out-of-network providers, but you may pay more for the same services.


        Out-of-pocket maximum: the highest amount of covered expenses that you will have to pay in one benefit period.


        Patient responsibility: the portion of the billed amount that is your responsibility.
